Make sure that your wishes are followed in the event that you become physically or mentally incapacitated, with this Personal Directive for the Province of Alberta. A Personal Directive is sometimes also called an advance directive or a living will. With this form, you can appoint an agent of your choosing (and alternate agents, if desired) to act on your behalf, and you can give your agent detailed instructions and set out your wishes regarding medical care, living conditions, personal and legal matters. Making a Personal Directive gives you and your family peace of mind.
