Use this template form to create a Treasury Order for use in Canada, from a corporation to a trust company or treasury agent, to issue fully paid and non-assessable shares to investors who are purchasing the securities as part of a private placement offering.

The share certificates must bear a legend stating that the shares cannot be transferred until a specified date, pursuant to hold period restrictions set out in the applicable securities legislation.
