Use this template to prepare a Form 20A (IP) Acknowledgement of Individual Purchaser, for use in the Province of British Columbia pursuant to a private placement offering of shares. This form must be completed by any individual person who purchases shares under the private placement.

The form states that the purchaser is aware of the risks involved in the investment, and sets out which Securities Act exemption the issuer of the securities is relying on.
