Use this template to create a custom Internet Subscriber Access Agreement between an internet subscriber (customer) and a service provider (ISP) for ISDN access and license to use the access software. Provisions include:

• fees billed on monthly basis;
  
• disclaimers on the part of ISP regarding content available on Internet;
  
• customer solely responsible to ensure it is not in violation of intellectual property rights of other parties in using content;
  
• ISP gives no warranties and provides the service on an as is, as available basis.
